Good Faith Estimate
At Mosaic Medicine, PLLC, a Good Faith Estimate is a written, upfront estimate of the expected charges for your medical care, provided under the No Surprises Act so you can understand your costs before treatment. If you do not have insurance or are not using insurance, you have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency Men’s Health & Testosterone care, including related costs such as medical tests, equipment, and other associated fees.
We will provide a written Good Faith Estimate at least one business day before your scheduled service or item, and you can also request one before you schedule care. This information is for patients who want clear pricing before treatment, how to get an estimate, and what to do if charges are higher than expected. If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill, which helps protect you from unexpected medical costs and make informed decisions about your care.
Please save a copy or take a picture of your Good Faith Estimate for your records. Our team will work with you to provide a comprehensive Good Faith Estimate detailing the expected costs associated with your Men’s Health & Testosterone treatment plan, answer questions about your rights, and help you understand the financial aspects of your treatment.
