What a family pays each month

Membership is a flat monthly fee for each person. There is no charge per visit for the primary care the membership covers.

Drawn to scale: each tile’s area is what that person pays a month.

Executive Direct Primary Care

$139a month for each adult

This is the base membership. It covers one adult’s primary care.

Adding a spouse

$115a month

A spouse joins at a lower monthly rate than the first adult.

Children

$50a month for each child

Children ages 3 to 21 can join. Their care is limited to sick visits. The section below explains what that means.

Each additional child

$50a month for each child

To join

A $100 enrollment fee and a 3-month commitment

You agree to stay for at least 3 months, so the first visits are not a one-time trial.

A worked example. Two parents and two children pay $139 + $115 + $50 + $50, which is $354 a month, plus the one-time $100 enrollment fee.

I'm not gonna lie, when I first came to Mosaic Medicine, I was tired and exhausted from some of the battles of life and personal setbacks I had been going through. But I can't even begin to say how amazing this place has been for me. They completely helped me transform my life and get things going again better than ever. Mentally and physically, I feel stronger, have more endurance, more energy, and just feel better getting through each day. They really took the time to analyze my labs, explain everything to me, talk with me extensively about what was going on, and help get me back on track. Now, two years later, I can honestly say I've never felt better. I can't thank Dr. Corey and his staff enough. Everyone there has always been extremely friendly and helpful. they even put up with me on the days when I may have been a little grouchy! They are Awesome! If you're thinking about going, don't hesitate. Go see them and check it out for yourself. You'll be glad you did.

I work 45-55 a week on average and have very limited time. Mosaic worked around my schedule for the initial consultation and even more so after the fact. Cory truly cares and helped me out when I was in a situation. If you’re even reading this review or the many others, look no further. The professionalism and quality of care is far superior to anything I’ve experienced previously.

I can honestly say that Mosaic Medicine is the best of the best. After my hysterectomy I was miserable. My corporate medicine doctors gave me multiple different pills and creams that never worked and then nothing. I was miserable and just couldn’t accept that this was going to be my new life reality. I did a couple consults with a few places and once I met Cory and his amazing team I was in. The office is clean. The customer service is top notch. This is patient centered personal care. Labs are done in house and your labs go into the portal AND you get a Tele-Health appointment to go over your labs. You are never rushed. When you are there it’s like you are their only patient. Your plan of care is a 2 way discussion, meaning you have a say in what your plan of care will be. My life today is 100% better than it was. Joining the Mosaic medicine family was the best decision I have ever made. I wish I would have done it sooner. They look at your health from every angle and develop a comprehensive plan for the new you. They see the potential in your health status and take you there. Thank you Cory and the entire mosaic team for being amazing from the moment you walk in the door. If I could give a higher than 5 star rating I would. The bed side treatment is the best I have witnessed and experienced. Keep setting the bar high!!

Mosaic Medicine is at the top of my list! The medical offices are clean and the area where you receive the IV is set up nicely with accommodating features including calm lighting ambiance and a TV whilst the infusion runs it's course. The Mosaic team is warm, welcoming and kind. I found the medical staff to be swift and efficient with the entire IV process, happy to answer any questions along the way. I am very pleased with my previous experiences here and look forward to my next appointment.

I have been seeing Dr. Cory for several years now and it has always been a good experience. I appreciate the time he takes to discuss your current health to determine if medication is necessary or if simple lifestyle changes can address the issue. All of his staff members deserve 5 stars as well. They all are so welcoming, friendly and have great bedside manner.

My experience with Cory and his team has been incredible. First for a long time I had time to talk, to explain my situation and to set my goals. Cory takes the time to understand and to find the good solution. The blood test helped to confirm everything. I have lost 16 pounds and feel so much better. I am on the right path and I feel really in good hands

Book a visit to join

Tick Direct primary care on the form. In the last box, tell us who in your family wants to join and what has been hard about your current care. The office confirms your appointment and answers your questions about joining.

By sending this you agree to be contacted by Mosaic Medicine about your appointment. Nothing here is a medical record.

A smiling female primary care physician in a white coat sits and talks with a young family, including parents and two small children, in a bright modern clinic with palm trees visible through the window.

Where

3934 FL-64, Bradenton, FL 34208. Always accepting new patients.

One provider who knows your family

Most primary care practices see a new face every visit. A provider in a large insurance-based group manages thousands of patients and has minutes for each one. Here the panel is small, so your provider remembers last month’s visit without opening the chart.

300

The most patients a Mosaic Medicine direct primary care provider ever manages.

3,000+

What insurance-based healthcare requires the average primary care provider to manage.

Both figures are the clinic’s own published numbers. For a family, that means one place to call when anyone is sick, and one provider who has the whole picture.

What is included in a family membership

Your membership covers the primary care your family uses most. Mosaic Medicine does not bill insurance for primary care, so there are no copays and no billing codes between you and your provider.

Long visits.

30 to 60 minutes with your provider, not a 7 to 15 minute slot.

Fast access.

Same-day or next-day appointments when someone is sick.

Direct contact.

Message or call your provider, or book a telehealth visit.

Everyday care.

Annual physicals, preventive screenings, chronic condition management and acute care.

Procedures and referrals.

In-office procedures, plus help coordinating with specialists.

The membership is not health insurance. It does not pay for hospital stays or emergency care, so keep a plan for those.

Care for children ages 3 to 21

Children ages 3 to 21 can join with a parent. Their membership covers acute care and sick visits: fevers, ear infections, sore throats, injuries that need a look.

Children under 3 are not accepted. Ages 3 to 21: sick visits and acute care

It does not cover growth and development visits or wellness visits. A pediatric provider should follow your child for those, along with developmental and cognitive screening, vision and hearing testing and vaccinations.

That split keeps each job with the right provider. We take the sick child on a Tuesday afternoon, and your pediatric provider keeps the schedule of checkups.

What happens when someone is sick at night or on a weekend

You do not reach an answering service. Calls after hours and on weekends go straight to our office manager.

Step 1

The office manager takes the details and contacts the provider on call.

Step 2

The provider replies by text or phone call.

Step 3

If you need a prescription, the provider can send it.

Step 4

The office manager books any follow-up visit.

You are not left to decide between waiting and the emergency room on your own. For a true emergency, call 911.

Is a membership right for your family?

A membership works best when you want one provider you can reach and plan to stay.

If you are not sure, book a visit and ask. We would rather tell you it is not the right fit than have you find out later.

It tends to fit if:

  • You are tired of a new provider every year.
  • You want time at each visit to ask your questions.
  • You want to book for a sick child without a long wait.
  • You are comfortable with a nurse practitioner as your primary care provider.

It may not fit if:

  • You need a pediatric provider for a child under 3.
  • You want only one visit. The membership has a 3-month commitment.
  • You want the fee to work like insurance. It does not.
  • Your plan requires an in-network physician for referrals. Check with your plan first.
